    Forms

    To lessen the time spent in our waiting room, please complete your new patient registration through the link sent to you via email and/or text message two days prior to your appointment, please bring your insurance card to be scanned at time of check-in.

    Late Cancellation & No-Show Policy:

    As a specialty practice serving multiple regions, we kindly ask that you review our cancellation policy. This helps us provide timely care to patients who may have limited access to Allergy & Immunology services.

    We request at least 24 hours’ notice for cancellations. If less notice is given, or in the case of a late cancellation or no-show, a warning letter will be sent as a reminder of our policy.

    • First no-show: Patients will receive a warning letter.
    • Second no-show or late cancellation: A $75.00 fee will be applied.
    • Patients with a history of multiple cancellations (whether late or with proper notice) may be subject to dismissal from our practice due to attendance expectations.
