Scheduling for Current Patients

Allergy Shot Patients:

  • Please be aware that all allergy shots must be scheduled in advance; walk-in appointments are not accepted. Make sure to arrive early for your scheduled shot appointment. If you arrive more than 15 minutes late or do not have an appointment, you will need to reschedule.

Current Patient Scheduling:

  • For established patients with in-person appointments, please arrive 10 minutes early. If you arrive more than 7 minutes later than your scheduled time we will need to reschedule.
  • Enjoy the convenience of booking your visits whenever it suits you! You can now schedule your allergy shot, extract refill, or recheck appointments online through our website at any time—just follow the hyperlinks on the right. If you are unsure of what type of appointment you need to schedule, please call our office for support (541) 683-1577 press 1 for established patient scheduling support.
  • Need to call the office or connect with your provider? Use the patient portal to send us a secure message. You can access the portal (My Health Record) by visiting the homepage and logging in.

Cancellation Policy:

As a specialty practice serving multiple regions, we kindly ask that you review our cancellation policy below. This helps us provide care to patients who may have limited access to Allergy & Immunology services.

  • We kindly request that patients provide at least 24 hours’ notice for cancellations. If less notice is given, or in the case of a no-show or late cancellation, a warning letter will be sent advising of our cancellation policy.
  • At the second no show or late cancellation a $75.00 fee will be applied.
  • Patients who no-show or cancel late twice consecutively will be discharged from our practice. Additionally, patients with a history of frequent cancellations will receive a warning regarding attendance expectations, and continued issues may also result in discharge.

Attention Aetna & First Health Patients:

Effective March 23, 2025, Oregon Allergy Associates and Praxis Health will no longer be contracted as an in-network provider with Aetna Commercial insurance plans.

We understand that changes to your healthcare coverage can be challenging. To ensure continuity of care, we recommend contacting your insurance provider to discuss in-network provider options available to you. You can reach Aetna’s customer service by calling the number on the back of your insurance card.

Please note that continued care after 3/23/25 will be considered out-of-network, and you will be responsible for out-of-pocket costs, which may be higher even if you have out-of-network benefits. We recommend contacting Aetna to confirm your billing responsibilities before proceeding with care.
