Want to become a New Patient?

Please call 541-683-1577 opt 2, to speak with a new patient coordinator.

 

What to expect after your New Patient appointment has been scheduled:

Once your appointment is scheduled, you’ll receive a link to complete pre-registration forms. These forms must be submitted prior to your appointment. You will be asked to confirm your appointment. If you are unable to complete pre-registration forms online, please arrive 30 minutes early to complete them in the office. If needed, bring someone to assist you in filling out the forms.

Arrival time:

Please arrive 10 minutes prior to your appointment. If you arrive more than 15 minutes late, your appointment will need to be rescheduled.

Checking In:

Upon arrival, check in at the front desk to notify staff that you are ready for your appointment.

Our Cancellation Policy:

If you need to cancel or reschedule, please provide at least 24 hours’ notice. After two cancellations or no-shows, we will be unable to accept you as a new patient.
