New Patients

What to expect after your New Patient appointment has been scheduled:

Once your appointment is scheduled, 2 days prior to your appointment you’ll receive a link via email, text, or both to complete your digital pre-registration forms. These forms must be completed prior to your appointment.

You’ll also receive a request to confirm your appointment via text or email.

On the Day of Your Visit:

  • Bring your photo ID, insurance card(s) (primary and secondary, if applicable), and prescription drug card.
  • Check in at the front desk 15 minutes prior to your appointment time.
  • If you haven’t completed the forms online, please arrive 30 minutes early to fill them out on our office iPad. If you need assistance, consider bringing someone with you.

Cancellations & Rescheduling:
If you need to cancel or reschedule, please provide at least 24 hours’ notice per our cancellation policy and to avoid any fees. (2) No-shows or late cancellations for a new patient appointment will result in our inability to accept you as a new patient. See our late cancellation & no-show policy below.

Pre-Registration Accommodations:
If you need to complete a paper packet instead of digital forms, please inform our New Patient Coordinator. We can mail the packet to you in advance or can arrive early and request to complete it in our clinic.

Late Cancellation & No-Show Policy:

As a specialty practice serving multiple regions, we kindly ask that you review our cancellation policy. This helps us provide timely care to patients who may have limited access to Allergy & Immunology services.

We request at least 24 hours’ notice for cancellations. If less notice is given, or in the case of a late cancellation or no-show, a warning letter will be sent as a reminder of our policy.

  • First no-show: Patients will receive a warning letter.
  • Second no-show or late cancellation: A $75.00 fee will be applied.
  • Patients with a history of frequent attendance issues may be subject to scheduling restrictions or dismissal from our practice at the discretion of their provider.

These policies help us prioritize patient care and ensure availability for those in need. We appreciate your understanding and cooperation.

Interpretation Services:

We offer interpretation services to support your appointment needs. Our available services include:

  • Certified Languages – Over-the-phone interpretation support.
  • All Hands ASL – In-person American Sign Language (ASL) interpretation.
  • Please inform our schedulers in advance if you require an interpreter, and we’ll be happy to arrange it for you!
